Your First Visit

When we meet for your first  appointment you will be asked to complete a comprehensive intake form. The intake form asks questions about your current state of health, past illnesses, and family history. These questions are important because the holistic approach of integrative medicine takes everything into account and an illness in one area of the body will ultimately lead to an imbalance in another.

After reviewing your intake form, we will discuss your condition, and the best modalities to proceed with your treatment in order for you to reach your optimal physical and mental health.
